If you qualify for Medicaid long-term homecare you may qualify for access to a full-spectrum of resources to help you live independently. The goal is to reduce cost to the State and keep New Yorkers in the most appropriate, least restrictive setting.
The NHTD program is for aged or disabled adults with Community Medicaid capable of living independently with one or more services. Your individual plan will cover services you need to live independently.
Who Qualifes?
The general requirements are:
- Adult 18+
- Requires nursing-level care
- Qualifies for Community Medicaid
- Chooses to live in the community/age in place
- Does not participate in another HCBS waiver
- Home is more cost-effective than a specialized facility
