New York Medicaid offers Nursing Home Transition and Diversion Waivers to help you make day-to-day decisions yourself, choose your own living situation, and reduce overhead care costs.
If you qualify for Medicaid long-term homecare you may qualify for access to a full-spectrum of resources to help you live independently. The goal is to reduce cost to the State and keep New Yorkers in the most appropriate, least restrictive setting.
The NHTD program is for aged or disabled adults with Community Medicaid capable of living independently with one or more services. Your individual plan will cover services you need to live independently.
